HomeConnect

Matching families with licensed I/DD care homes

HomeConnect homepage: find a care home that fits, before you tour, with home search form

Overview

A search and matching platform that helps families find licensed residential care homes for people with intellectual and developmental disabilities, showing which homes have an open bed and accept their funding.

The problem

Public care-home directories are often out of date, so families make dozens of calls and fill out the same intake packet again and again, only to find there's no open bed.

What I built

  • Home search by location radius (10–500 miles or current location), population served and funding program
  • Listings with open-bed status, accepted programs (HCS, ICF/IID), care services, staff qualifications and licensing
  • A single care profile families share with homes instead of separate intake packets, with alerts when a matching bed opens
  • Tour invitations that homes send and families accept or decline, with email notifications
  • Encrypted sensitive data (diagnoses, clinical notes, SSNs), 10-minute expiring document links and a full access audit log
  • Provider tools for listing homes, keeping open beds current and embedding a search widget
